Mediterranean Keto: The Complete Guide
Editorial team · 5 min read

A traditional Mediterranean meal is often built around olive oil, fish, vegetables, herbs, and fresh cheese. A keto meal is built around protein, fat, and very few carbs. These two styles overlap more than you might expect.
This guide shows how to combine them: what to eat, what to swap, a sample week, and where to start with recipes. If you are new to keto, begin with Keto Diet for Beginners and the keto food list. Our Mediterranean section collects all the recipes in one place.
This is general education, not medical advice. If you have a medical condition or take medication, talk to a doctor or registered dietitian before you change your diet.
How Mediterranean and keto fit together
The Mediterranean way of eating is not one cuisine. It covers Greece, Italy, Spain, southern France, Lebanon, Turkey, and North Africa, and it leans on a shared set of ingredients.
What they have in common with keto:
- Olive oil as the main fat.
- Fish and seafood, which are naturally low in carbs.
- Eggs, poultry, and lamb.
- Vegetables such as cucumber, zucchini, eggplant, spinach, and tomatoes (in moderation).
- Olives, feta, and yogurt-based sauces.
- Herbs and lemon for flavor instead of sugar.
Where they differ:
| Traditional Mediterranean | On keto |
|---|---|
| Bread, pita, pasta, couscous, rice | Replace with vegetables, salads, and extra protein |
| Legumes such as chickpeas and lentils | Too high in carbs for most keto plans |
| Fruit, dried fruit, honey | Limit to small portions of berries, skip honey |
| Potatoes | Replace with cauliflower or zucchini |
| Sweet pastries | Replace with keto desserts |
The idea is simple: keep the flavors and the fat sources, and swap the starch.
A note on health: Mediterranean eating is often associated with good heart health, but that does not automatically carry over to keto. Keto changes cholesterol differently from person to person. If you have heart or cholesterol concerns, ask your doctor to monitor your blood work.
What to eat
Proteins
- Fish and seafood: salmon, sardines, tuna, cod, shrimp.
- Poultry: chicken thighs and drumsticks.
- Lamb and beef: grilled, roasted, or as meatballs.
- Eggs: frittatas and baked eggs.
Examples: Mediterranean Baked Salmon with Olives and Tomatoes (about 2.3 g net carbs per serving), Greek Shrimp and Feta Skillet (about 7.3 g), and Greek Lamb Meatballs (Keftedes) (about 1.7 g).
Vegetables
- Cucumber, zucchini, eggplant, spinach, green beans, asparagus, and leafy greens.
- Tomatoes and onions in small amounts, as flavor rather than the main part of the dish.
Examples: Mediterranean Chopped Salad (about 3.4 g net carbs per serving) and Greek Lemon Roasted Zucchini with Feta (about 6.7 g).
Fats and flavor
- Extra-virgin olive oil, olives, and avocado.
- Feta, halloumi, and other cheeses. Baked Feta with Olives and Tomatoes is about 3.6 g net carbs per serving.
- Tahini in small amounts.
- Lemon, garlic, oregano, mint, dill, parsley, and capers.
Dips and starters
Yogurt-based and vegetable dips make good snacks with cucumber slices. See Tzatziki with Cucumber Slices (about 6.3 g net carbs per serving) and Roasted Eggplant Dip with Cucumber Slices (about 6.9 g). Check portion sizes, because yogurt and eggplant contain natural carbs.
Olive oil on keto
Olive oil has no carbs and fits keto well. It is also very energy-dense, with more than 100 calories per tablespoon. If you are managing weight, measure it rather than pouring freely. Use extra-virgin for dressings and finishing, and any good olive oil for cooking.
What to swap
| Instead of | Try |
|---|---|
| Pita with souvlaki | Lettuce leaves, or just a salad. Try Greek Chicken Souvlaki with Tzatziki |
| Rice or couscous | Cauliflower rice, or a bowl of salad. See Greek Lamb Gyro Bowls with Tzatziki |
| Pasta | Zucchini noodles |
| Potatoes | Roasted zucchini or cauliflower |
| Chickpeas in salads | Extra olives, cucumber, or feta |
| Honey and syrup | Lemon, herbs, and a keto-friendly sweetener. See Keto Sweeteners and Baking |
| Baklava and pastries | Vanilla Panna Cotta with Berries |
Greek food on keto
Greek menus offer a lot of keto-friendly options: grilled meats, souvlaki, salads without croutons, tzatziki, olives, and feta. Skip the pita, rice, potatoes, and anything glazed with honey. When eating out, read our dining-out guide.
A sample week
This week uses recipes from this site. Net carbs are per serving from each recipe page. Daily totals are the sum and vary with portion size. Add vegetables, a side, or a snack if you need more energy.
Snacks and sweets
- Baked Feta with Olives and Tomatoes, about 3.6 g net carbs per serving.
- Prosciutto-Wrapped Asparagus, about 2.2 g.
- Vanilla Panna Cotta with Berries, about 4.3 g. It contains gelatin, so it is not suitable for vegetarians.
Tips for the week
- Make the frittata or the spinach pie once and eat it over several mornings.
- Batch the tzatziki and keep it in the fridge for a few days.
- Use leftovers: souvlaki chicken goes well on a salad the next day.
- Check the sodium on each recipe page if you watch your salt. Feta, olives, and capers can be salty.
